Dark eye circles are often a result of tear trough deformity. However, the causes of dark eye circles can be multifactoral. Changes in the skin thickness, laxity, hyper pigmentation and actinic changes also play a role.

The tear trough deformity is a major concern in a lot of individuals seeking periorbital rejuvenation. A prominent tear trough deformity is characterized by a sunken appearance of the globe that results in the casting of a dark shadow over the lower eyelid, giving the patient a fatigued appearance despite adequate rest, and is refractory to attempts at cosmetic concealment.

 

Thin skin or prominent subcutaneous venous pooling accentuates the periorbital darkening. Additionally prolapse of the orbital fat may indirectly cause a shadowing over the lower lids.

Dr Chew prefers to use a low viscosity HA  dermal filler like Restylane Vital Light to correct the tear trough deformity.

 

The technique used is essentially a multi-layer technique ( MLT ) with most of the filler injected in a deep plane below the muscle and just above bone (supraperiosteal). A small amount is injected at a more superficial plane and gentle massage is done to ensure a even distribution.

 

Over correction should be avoided as HA is hydrophilic and may have a swelling due to its property of attracting water.

The results are expected to last on average about 9 to 12 months.
